Damien has raised over £1200 so far for his triathlon fundraiser - but we can do more!

YOU can help, by asking your work colleagues, school mates, friends and family to sponsor Damien, using the attached sponsorship form

You will need to:

1. Print out the sponsorship form by clicking on the link at the bottom of this page, then selecting the printer icon at the bottom right 

2. Ask people to donate

3. Collect the money and add the total amount to Damien's fundraising page

PLEASE NOTE: you will need to add the total amount yourself using your credit card or your Paypal account, or if you do not have a paypal account or credit card please be sure you have someone who is able to do this for you before you begin!

4. On the donation page be sure to write where the donations came from in the message box ( e.g donations from my work mates £1 each from Jane, Jim and Jack, £5 Mr Jolly, £20 Ms Jemima etc...)
